Build a competition entry mechanic module for DNN (DotNetNuke) -- 2

Create a competition entry mechanic module for DNN (DotNetNuke). User will be presented with a form to capture information: First Name, Last Name, Phone Number, Unique code, check box for agreeing to terms and conditions(link to Terms and Conditions page, configurable in module settings) Upload Button to allow the user to upload an image, Google recaptcha, and submit button. Unique Code a csv file will be provided with unique alpha numeric codes (allow for up to 500, 000 possible codes)and will need to be pre-loaded to the module via module settings, when user enters a correct code they will be able to submit the form, if the code is incorrect or has previously been used they will not be allowed to submit the form and will be shown an error message. Require field and reg expression validators will be required for all fields. failure to meet validators will not consume the unique code. Each successful submission of the form will add the user to the database and will represent one entry. The user will also have the option to upload one image file with their entry. All image submissions will need to be retrieved on demand by website admin and also emailed to an address defined in the module settings. At the completion of the competition the site admin will need the ability to export the entries as a csv file. this will contain all form fields. Success message this will need to be configurable by admin via module settings with the standard DNN WYSIWYG editor. Design the successful developer will be provided with a PSD of the initial state of the form (before the user fills it out) and you will use this to layout the form and also use as a guide for any further success screens etc. Please quote reference #CompMech2 in you initial correspondence to show you have read the details of this build.
