I can provide you with links to several enrolment forms on the web. When a user completes one of those forms and submits it, I also want the information from three of the fields (Name, Email, Location) collected in a very basic database.

Hi, I can make you a form for your website that will collect the fields to a database when the user submits, and I can also have it e-mail you if you would like.
