Create PDF with editable form fields from Word document

Attached is an employment application in Word (Employment Application for Drivers - For [login to view URL]). I would like this form to be converted to a PDF that has "form fields", so the applicant can enter their information directly on the PDF. I've attached a PDF, [login to view URL], to show what I mean by this. In the process, I also ask that you clean up the form some, to make it look better than it current does. Right now some of the spacing, fonts, alignment, etc is not ideal. In general, while making the conversion please pay careful attention to making the final form look good, clean, logical, PROFESSIONAL. I will be picky about this. I need a worker with good "attention to detail". Some of the text is highlighted yellow in the Word document. I do not want the PDF version to have the yellow highlighting, so please remove in the conversion. The highlighting is just to differentiate certain text. See below. I want THREE variations of PDF form: 1) All the text included 2) Make another PDF, but with the text/parts in yellow excluded (do not simply cut/paste -- you need to still make it look nice and professional) 3) Make another PDF, but ONLY with the text/parts in yellow (again, it must look good) Lastly, I also have a logo to add (inserted image) to the Word document. I'll send the final version with the image added before you start work. Hopefully this should not be an issue in the conversion to PDF. I do not believe it should be. thanks.
