Design & layout for survey form #2

Designer needed to lay out a branded survey form - so it has a professional, polished and organized appearance. ## Deliverables *** This is a print design project. Final survey will need to fit within 5.5 x 8.5" space, allowing for 1/4" margins. *** We will provide a Word doc with the survey text and questions, along with a JPEG for the company logo, fonts and a rough sketch. We need design & layout applied to this, so we end up with a form that both reflects a polished and professional appearance for the company, and is organized and user-friendly for those who are performing the survey. The final layout should reflect a document one would expect to see from a well-established high-end organization. The final deliverable will include the native files, formatted in MAC compatible InDesign or Illustrator CS5 or older (Word or Pages is also acceptable, though not preferred); and a desktop-printer optimized PDF. The document can use color (for logo, etc.).
