Design a Website Mockup

Hi. I need someone to design a Responsive restaurant theme/template for me. The design will be written using HTML5/CSS. All design and images used must be free for commercial use and not copied. I would like a design similar to [login to view URL] but obviously not the exact same. I would also like a working contact form for this theme/template. I am providing some images that can be used in the design but you can also add some if you feel they are good quality and they are free for commercial use. This theme/template is a test to see if i can find a good designer for future projects.
