Create a Landing Page with Contact Form

I need a landing page designed and implemented into a new wordpress installation. I have attached the flyer that has all the information that needs to be included and 4 photos of golf courses that need to be used on the page. This is a one page site that will be used as the landing page for Google Adwords ads. The page content will include all of the information in the flyer in the left side of the content area and a short contact form on the right hand side of the content area (name, phone, email, number of people, date range of stay, comments) and a "click to call" phone number prominently displayed on the page. The page title and heading 1 will be "Golf the Best of West Michigan" with a sub-heading reading "Golf Packages Available at Group and Corporate Rates." The page design should include a full width background image of people on a golf course with a "boxed" content area including the golf package information and photos of the 4 golf courses with names of the courses and logos embedded in the photo or as captions. My total budget for this project is less than $100. Bids more than that will be ignored. I am looking for a provider who has experience making professional landing pages. Please submit a list of landing page examples you have created that I can view. Winning provider will be chosen based on price and experience. Cpanel and Wordpress admin access will be provided to winning provider upon acceptance.
