Crop PDF file

The attached PDF file is 9X14.5 cropped to 8.5X11. I need just the cropped protion as a separate PDF. Quality and positioning must remain the same. I have Adobe Acrobat 5.0 (full install), but cannot find how to get this done. What I really want is for someone to spend 15 minutes with me on the phone so I can do this myself next time. I posted this project on several sites. My problem has been solved.
