create and embed custom form widget

I have a website - [login to view URL] (Wordpress). I need to add a form widget towards the top of this page, probably to the right next to my you tube video, where a person can enter their name, company name, and email address and then download a one page word document. I'd like the widget show/say (Free Download - Safety Policy Sample) and have a pic of the doc or something enticing like that). And example of a widget layout I like is: [login to view URL] - The widget I like is on the right side, titled "Want a Free Experience Mod Review?" Please type "insurance is fun" in your job reply for consideration of being awarded the job. P.S. I use gravity forms right now for my contact me page.
