Custom Lead Tracking Software, DNN Module would work as well

Hello, I need a custom lead tracking application written in ASP.NET in vb.net preferably. ASP.NET 1.1 or 2.0 either one would work fine. IF you know how to make it into a dotnetnuke module then that would be the best. Here are the requirements: Here are the details: 1, Telemarketer makes the lead 2, Telemarketer gets all the customer information, e.g. name, address, state, city, zip, phone (textboxes that include validation) 3, Question of who is the home owner 4, Whether they are married or not 5, Asks what products/services the customer is most interested in (a Checkbox list of products, including an “other” box which will also have a textbox where they can describe what it is in detail…) 6, Asks what is the best appointment time 7, System records what time & date the lead was made on Now the approval process 1, Administrator verifies the lead by calling in 2, Assigns a sales man to the particular lead 3, Tracks status of the lead 5, Closes lead if it went on to a sale or if it was a pitch or if it was a dead lead 4, If the lead is a sale, admin specifies whether the lead was a Cash Sale or a Finance job and the amount 6, Tracks which sales man completed the lead. Then we need reporting options: 1, Reports by total sales cash sales or Finance 2, Reports by sales made by which salesman 3, Sales reports by product type 4, Lead analysis: which telemarketer made the lead, around what time and date, how many leads where dead, good, pitched, or lead to a sale. If you can not do dotnetnuke based module, then the application needs to have a skinable interface. THIS PROJECT IS WORK FOR HIRE. The backend needs to be SQL Server based, preferably SQL server 2005 express and use stored procedures. Will be willing to pay up to 1500 for a really good customizable dotnetnuke module that lets us add/remove feilds. The max we can do for a non dotnetnuke module is about $1000.00
