adding data from a layout to mysql and CMS

have a PHP layout, I need to a layout before the customers submits the layout data. if customer clicks on the "Submit Button", all data should go into MYSQL database tables. The admin as well as the user, must receive customized e-mail notification upon the submitting of the form. I need also a page for the admin, to show the customer's data along with the order number, and the data fields on the admin page should be editable. All pages must pass the HTML5 validation with no errors. person should highly qualified for the AJAX, HTML5, jQuery , MySQL, PHP
