Add upload function to existing PHP script.

Hi, Please review the following script, which is a URL shortener: [login to view URL] I'd like to have this customized for my personal use, so that I can upload a file to my server, and have a shortened link produced once the file upload process is complete. The shortened URL will of course direct to eh file being uploaded. I want files uploaded to a "/files/" directory. Please add the upload form code at the bottom of the existing index page that is included in the script folder. You can download the script from the link I provided above. I will provide a 10 rating for fast work.
