Class Library Integration into Sample Form

This project is for 2 hours of work at $8 per hour. If it takes you less than 2 hours (which I suspect it will), you still earn the full two hours' pay. The task is integrating a PHP class library with an HTML form. The form is very simple, and the requirements of the form are simple. Because the script is for demonstration purposes and the task is easy, the budget is low on this project. The full project details are attached as a document.
