Create the terms page for each of client;s programs, with their logo on top and accept button on bottom (5 in total). 2. When the user clicks the “I agree to the terms, load the form” button, the light box pops up 3. Client will have an excel sheet with the questionnaire details for each program 4. Client has 4 services (3 different programs for each) plus the website service 5. There will be a main questionnaire for each service, and each will just need minor changes to it depending on the program details 6. At the end of each questionnaire, client would like a button “Submit Information & Pay” and a button “Submit Information – Already Paid” 7. The first will take them to our paypal payment for each program. No payment button required for the website program – just a “Submit Information” button 8.
