Configurable form

Need to do this in less than a week. Goal : Create a configurable html form in php with text free answer, radio button answer and multiple answer questions. The database is mysql. Later will upload it. Details I have an app made in vb6, that already do this, I will deliver the source code and database modeling so you can get the logic from there and start the job from this point. Database detail . It’s a tree view, because a question can have a parent , that is a reference to another record from the some table. The question type can be : Livre (free): the user can answer with a text, use the field lines to define the field heigh Sim / Não (yes/no) : the answer can be yes / no. Each answer is of type texto. Múltiplas Opções (multiple choice) : the answer can have multiple answers to choose. Each answer is of type texto. Texto (text) : type used in the options of Sim/Não question and Múltiplas Opções question. The form will read the parameter cod_grupopergunta and show the questions.
