Auto-pilot mailing list script needed

The script will need to have the following: -User enters 10 digit cell phone number in the box and has a drop down list after that has cell phone providers. The user selects the one they have and the script automatically adds the ending corresponding to the provider they chose. For example, if the user chose Verizon as the provider it would add to the mailing list. -User selects a specific time (no minutes just on the top of the hour) and time zone they wish to receive the daily email. -I must be able to enter my emails for days in advance for them to automatically be sent out. -The script automatically sends out my email to all users at the time specified each day, when they signed up. -An unsubscribe needs to implemented as well so users can visit my site and unsubscribe to my mailings. -The code for the form must be easily entered into an existing layout for my site. This script basically needs to be 100% auto-pilot so I can focus on other projects. I am looking to spend no more than $200 on this. This is not a hard project so high bids will not be accepted.
