Create PHP / MySQL party order / reservation form with backend

I need to create a party order/reservation form. I already have the design and need help building the functionality and back-end. The front-end is already built for the most part, but needs modifications. I have a working backend that you can use to create the new backend. This will help you a lot and save a lot of time. All FTP and database access will be provided once the coder is selected. IMPORTANT: All work done will need to be documented adequately. All settings and admin functions must be fully documented. ## Deliverables FRONT-END: The front end is located at the following URL: [login to view URL] The date needs to be a pop-up calendar that allows the user to select a date. Once the date is selected, it will be populated in the Date field. The time needs to have some kind of cool time selecting function. U am open to suggestions on this one. The number of guests needs to be a pull down menu in increments of 1. It needs to go from n to n+m, where n is the minimum number of guests and the n+m is the maximum number of guests. Both "n" and "m" need to be easily modifiable through a settings file or the heading of a common PHP file for example. The order form will have two options: lunch or dinner. For each option, people will be able to choose appetizers, entrees and desserts. If a person orders lunch, they will be able to choose 3 of each category. If they order dinner, they will be able to choose 4 of each category. All food items will be represented as check box, two column display. The pull down menus you see on the form are for testing purposes and they will need to be removed. The form will display the names of the food items. Each item has a description which is to be displayed in a small pop-up (or bubble) when the user hovers the mouse over the item name. All food names and descriptions and information about lunch/dinner and appetizer/entree/dessert is included in the attached spreadsheet. The system must display the correct food items for lunch or dinner. The system must limit the number of selections by the user via JavaScript of AJAX, etc.. So if the user chooses lunch, the system must display the correct items as per the Excel spreadsheet. All food items are to be stored in a database. The backend must have a page to edit/add/remove food items. The price will need to be computed automatically and added up based on lunch or dinner selection. The prices are listed on the form. Once the sub-total is computed for lunch or dinner, the system will need to add the tax and the 20% gratuity. The grand total must be displayed on the bottom as shown on the design. Note: There is one special case in Entrees. If Steak Frites is selected as one of the entrees for lunch, $6 must be added per person. There is a comment about that on the form. There are now three lines at the bottom of the form as follows: Sub-total: $0.00 Tax(8.875%): $0.00 Total: $0.00 We need to add one more line so that it looks like this: Sub-total: $0.00 Gratuity (20%): $0.00 Tax(8.875%): $0.00 Total: $0.00 All these numbers need to be calculated by the system automatically. All prices, taxes and gratuity need to be modifiable through a settings file or a PHP header file, etc... Here is how this new form should behave and function: [login to view URL] Place a test order and hit submit. You will see a confirmation page that lets the user review and then confirm the order. Once the order is confirmed, a thank you page is displayed. Our new system needs to generate similar confirmation and thank you pages. On the thank you page, the user will be able to print the order or go back and create a new order. If the user chooses to print, a printable pop-up window will appear and the print dialog is loaded automatically. A print button will also be provided as in the example provided. Also, a summarized copy of the order must be emailed to the person who placed the order and the site administrator. The email address of the site administrator must be changeable from the back-end or a settings file. BACK-END: The back-end pages will be protected with a password. On the back-end, the system will display a list of orders. We will need to decide how many are displayed per page. The order view page will need to look like the one in the attached picture. All functionality need to be duplicated: 1. A refresh button (text) to refresh the current screen. 2. An exit button (text) to logout. 3. When an order is placed, it is marked as PENDING. Once it is processed by the admin, they click the PENDING text and the status changes to COMPLETE. 4. The remove text will delete the order from the view. It will not delete it from the database. It will be marked as DELETED and hidden from the admin view. 5. View & print will load the order in a pop-up similar to the front-end print function. The other part (page) of the back-end will be used to modify the food items database, prices, etc... Please note that the above specs are general and may be slightly modified and/or appended during the development phase.
