App to let voters email their elected officials a form letter

This is a simple php application and part of the work is to install it and test it on my shared web server. I will favor people who respond early, have good ratings and promise to finish in a few hours while I wait. The app will consist of a page with a form letter in a text box which users can send as is or can customize. The page will also have check boxes that the user can select. Each check box will be next to the name of an elected official. If the user checks the box, then a copy of the email addressed to that official, will be sent to that official's email address. If the user checks several boxes, the same email will go to each, except the greeting will be different on each. For example, if the users checks the box for Mr. Smith and also checks the box for Mrs. Jones, then the email in the text box will have the body slightly changed. The first email body will start as "Dear Mr. Smith," and the body of the second email will start as "Dear Mrs. Jones," I must be able to enter the names and email address of up to 40 officials. The user's email must be given and must be validate to end with: gwu.edu. The layout of the page should be nice with nice fonts and well spaced check boxes, text boxes, etc. Obviously there should be the ability to change the subject line of the email. I would like some way to audit how many emails have been sent and which users sent what email to which officials. Perhaps you can capture this info in a simple MySQL table. This project requires installing and testing the application on my webserver. I need this ASAP. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform LAMP
