Arabic / English wordpress form needed

We need a form created for WordPress. The form will have three stages. Each stages will have different input and option field. The form will be in two languages Arabic and English respectably. The English will have Image right hand side and Arabic on left. Each stage of the form will have different picture. Please check attached Layouts for the design of the form. There will be one Participant field where user can add his/her name and DOB. The + icon is to add extra fields for extra user. There is a Print/PDF button which will pop-up a PDF with submitted data for user to print. The form to send an email with the form content submitted We have already WordPress site created and configured in both languages.
