CREATE PHP/JS/AJAX SCRIPT THAT CONVERTS INFORMATION FROM DB

I would like a script written in php / JS / AJAX that culls information from a database. Where there is ZZZZZZZ|AAAAAAA|PPPPPPP simply fill in the blanks from the text file I have uploaded. What script I want: When someone enters their AAAAAAA information, it is processed looking up info from a MySQL DB. The design of the DB is quite simple. Once they click "Create ZZZZZZZ" the screen then changes that into proper ZZZZZZZ for them to use on their website. 1. You will need to create the database of ZZZZZZZ rules, possibly from ZZZZZZZ website 2. After the visitor enters their AAAAAA or PPPPPPP info, they will then need to solve Captcha code to view the completed ZZZZZZZ information. 3. It will appear in a box where they can copy. I am only looking fro JS/AJAX/PHP coders who understand what type of script I would like to build.
