Create Accro Fields For PDF Forms

Create Accro Fields for PDF forms with meaningful names. I have similar forms for each year. For the year 2011, I have 2 forms: Forms 1096 & 1099. Form 1096 - One page of PDF document needed. Form 1099 - One Pdf document with 5 pages is needed. (All 5 pages are duplicate and almost exactly the same). I may have exactly the same forms for the older years. For example year 2010 or 2009 or 2008...... going back to 10 years or even more. So each year has 2 forms with a total of 6 page (as explained above). This makes a total of 12 pages (i.e. 4 forms - i.e. Two forms for each of the 2 years i.e. 2010 and 2011). Please bid on 15 similar pages. There are minor differences between the older versions. I like the field names to be the same for all the common fields for all these forms. When designing the forms, we like the ability for the end user to: a. print "data only" on a pre-printed form This is important and will need some coordination and testing.
