Web site survey comprised of: 1> 5 static Web pages 2> A survey containing 20 questions 3> The survey responses would be available for download in an Excel file via a password-protected Admin location. 4> The site would be hosted on a shared server with the domain name [login to view URL] ## Deliverables I am developing a web site that would have the following features: 1> 5 static pages. (see attached for samples) a. Homepage b. Contact page c. Privacy page d. Consent page e. Directory of Brand Measures page 2> A dynamic survey. The survey will prompt users for responses in the form of: Text box entries, Multiple Choice (1 answer only), drop down selections The survey would have a maximum of 20 questions. There would be 3 or 4 questions that would be “smart questions?? . For example, question 5 is, “Do you measure THE brand??? and prompt the user for either a yes/no response.
