Create, process, access and print to pdf online html forms

We need a site that allows for, 1. Select an html form from a list. Coder will need to convert existing paper forms to html? forms with images & text (3 forms which are quite similar). Form definition will be provided, please ask. 2. Allow entry of text data in the form. 3. Form page will have "Save", "Print", "Home" and "Saved Forms" buttons. "Save"will create pettifo files from the forms. PDF files should be saved on the server. File names are based on a predefined conventions. "Print" will save form similar to "Save" and popup print dialog. "Home" will bring up the list of forms. "Saved forms" will display the list of saved forms. 4. Saved Forms will allow access to saved pdf files as a list. Clicking on the file will download the file. Platform is Linux. Coder can use lpdf, iText or any other library to create pdf files. Any other approach is also acceptable after approval. Coder will need to get the design approved before coding. Coder will need to provide full install instructions.
