Custom PHP Form Development

What I need is: I have a form I created the layout for as far as inputs and all form elements and css. I am looking for someone to code the form in PHP. This for will be used as a learning tool for myself. I have to build forms from time to time and I and not really a programmer so I will be using this to learn how to build email and contact forms from. I am attaching a sample html form with the main elements that have to be in the form. If you see something that you believe I left out fill free to add it in or make suggestions whether its missing elements or a better structure and css from what I have so far. Heres a list a things I need the form to be able to do: 1. This form needs to be coded in PHP and totally contained within one page no separate possessing pages. 2. Validation for required inputs as well as validation for correct email address and phone numbers. 3. I need it to have the option to send the information collected to one or more email addresses. 4. When a user submits the form If errors page is to refresh and show errors next to each form element as shown in the attached sample without making the user refill in information that was entered correctly. 5. One of the option in the form it a file input that will be used to send along files as attachments with the email. I need this to support zip files and image files such as jpeg, gif, eps, png ect.. 6. After the form has been submitted I need the form to refresh and show a personalized conformation message. 7.I also need some basic commenting to show what the form is doing so that I will not get lost in your coding style. 8. Last important feature need is to have the option of sending a zip file or pdf on my server with confirmation to the email address they entered into the form field. I need this a.s.a.p. I do allot of work for non for profit business and I have to build some complex form on some of there sites and at the moment this is needed within a couple days. I am current at a stand still on some pro-bono non for profit work because of my lack of experience with forms. ## Deliverables I made mention to the file upload supporting an array of image types if its easier I will be fine if it only supports zip files.
