A couple of forms and a login

I need a login/password for clients that i set up. For instance, if you are my client and I am working on a job for you, I will give you a password and username to use to login to see your project. I will need access to create a random user and password and then I will deliver it to my client. I also need a contact form. Basically a form with name, phone number, city, and email address. Also need a box for comments or instructions that will consist of basic questions, like: Type of project, budget, deliverable time, already gotten other quotes", etc. I also need a box (already created) on every page (already on every page)? coded in php. Simple (name, email and comments). All of these should be done in PHP and are easy. There are no strange or unreasonable requests for this project. Straightforward. I am including the zip files.
