Debugging ActiveX comp for HTML

I am having some problems with my VB ActiveX code that I was hopeing someone would no how to solve. I have attached the zip files with all the necessary files. Now my question is: if you simply run it in design mode (F5) it works fine. However, I am not able to get a functional HTML file that will display the activeX. I set all my activeX security to enable, but that didn't seem to fix anything.
