Add a Store Locator to a website

I would like to add a store locator to an existing website that will show nearby stores when a user performs any of the following: -inputs a zip code -inputs a city and state -selects just a state from a drop down list. It should be integrated with Google Maps as well. Here is more info on making a store locator with the Google maps API: [login to view URL] I would like the ability to add multiple stores at a time by uploading a spreadsheet of some type AND I would also like the ability of adding the store info one by one into the system for single entries.
