Build a Webpage With Form

I need a webpage made for an accident claims company. The webpage needs to look commercial in its layout. There needs to be a form on the webpage that visitors fill out and submit. The submitted data must export to some sort of list I can access on the backend or must send to me via email. I will provide the majority of the content for the page including: -The Logo -The Copy Text -The Form Fields -2 Graphics that may be included. You do not have to use both images. You might choose to use just one or none at all, and you may resize or modify them as you wish. The main priority is that the webpage looks good. If you would like the best chance to be chosen for the job you will be able to provide me a preview of what the site will look like before I accept your bid request.
