Copy 5 Page Order Form to My Site

To place an order on my site, clients must click thru to the manufacturer's site to enter the order. I want to duplicate the order forms on my site preserving the pricing & options, but instead of the collected data being transmitted to the manufacturer, I want the data sent to me so I can enter it. I can use Dreamweaver to upload the modified pages, as well as modify the existing hyperlinks to target the modified pages. I also need to be able to adjust pricing & product tables in the future. I simply want the form results, shipping calculation & purchaser's personal info saved to a file on my webserver with e-mail notification of the order being placed.
