Create A Dreamweaver Survey Landing Page (Php, Photoshop, Dreamweaver) Out of 2 Pages I created

I want to create a Survey Style Landing Page, the size of a pop up. So what I mean by Survey Style is this: A website that read something like this: -------------------------------------------------------------------------------------- "Do You Want A Free Bottle of Weight Loss Pills?" Choose One of the below: Product 1 ---> Tick box Product 2 ---> Tick box Product 3 ---> Tick box Click Here TO Receive The Free Offer. --------------------------------------------------------------------------------------- All right, that's it. As you can see, choosing between product 1 and 3 means that there is some Php involved. I have the code. The above would look pretty bad. So I Have a landing page design, too. So basically, I have the design and I have the code, but those are two seperate files. All you need to do is to fuse them together to create a new landing page that has a nice design and php code that works. Also I need a full documentation as to how you did the task, so I can learn how you did it. and also i might have a few questions regarding that.
