Add Function To Existing Website

I currently have a website that allows visitors to put information into a form and then submit it to me. Once I get it, I have to copy the information to a custom template/web page. I want someone to modify it so that the form submits the info directly into the template, and holds all of the information in a database. At the present time, it does not use a database. There are a total of 20 different form that need to submit directly to 20 different templates. I am attaching a sample submission form and the custom template I want it to post into. All the other forms are only slightly different, so it's NOT a lot of work. In addition, as you can see from the example I attached to the project description, each submission form, (in this example, "dogs"), has a only ONE template with three spaces to go with it. Consequently, when the first template is full, the system will need to AUTOMATICALLY REGENERATE the template when a form is submitted. As well, it shouldn't post to the site until all three spaces are full. If we need to, we can change the template to just one space, but I still want three spaces per page as in the template example. Basically, what I'm saying, is that I don't want ANY BLANK templates to post to the site. And, last, but not least ...I bought a Membership Management Script for this site this morning that I will be integrating into the site, (I haven't downloaded it yet as it is 354 MB, so it'll take a full 24 hours by dial-up), but it allows you to add any amount/kind of custom pages to each person's member area, so it's very flexible. That said, I need the system to keep track of everyone's posts to the site, and allow them to view and edit those posts in their member's area. So, once the Membership Script is in place, you'd have to code the site so that a copy of each member's posts goes to their member's area. The Membership Script itself will keep track of everything else I need to monitor, so that's all I will need. Please remember ... I will NOT pay for incomplete work. However, I will be understanding and patient if it takes you longer than you quote me. What's important is that you are flexible and that you work with me on any "kinks" that may come up.
