Build a custom report writing platform

I want to have built a platform for writing medical evaluation reports. When the therapist performs an evaluation of a new patient, it usually takes a great deal of time to write the report summarizing the evaluation. We would like to find a way of building a database of standardized terms, tests, test descriptions, diagnoses, and other reusable elements of the report that can be added by a simple click. The report would also have to allow for typing in specific information such as patient name and demographics, as well as the specifics for each particular patient. The report would have to be formatted so that in final form it is a very professional-looking document. Also, since the users are therapists and not very tech-savvy, the platform will have to be very easy and intuitive to work with. We can provide much of the content (test, descriptions, diagnoses, recommendations, etc.), or at least tell where it is. If solid enough, this platform may be turned into a sellable product.
