Hi Moonbreakker, I've worked for the UK's largest retail banks (3 in one group) for the past 10 years, building databases and reporting solutions for them. Therefore, although my experience on Freelancer.com isn't very large, I'll be more than capable of building the tool you want. I use Microsoft Access 2013 most, but can build it in any version you need - best to match the software you currently have. Tables will store all of the information you need, forms will make it easy to use and show the patients photo (my assumption is that this is to verify their identity). An alternative would be to have an online database - the same solution as MS Access, but accessed via the web from any location. All the best, Ben
