Create SQL database linked to web page for directory

Need help designing a web directory. On a simple google-like landing home page with google ads, users will have an opportunity to search for certain terms and descriptions in our database, or to enter certain terms and descriptions into our database. You will design the landing page (with spaces for searching, spaces for entering data, and google ads) and the database. I am currently hosting on Servage and will provide FTP access. Servage has a mechnism for creating a SQL database. I will also need a mechanism to review terms that user enters before releasing to the database to weed out bad entries. That's about it. You will need to sign a confidentiality agreement before we start.
