Please refer to the forms currently coded here in html here: [login to view URL] If you look at the drop down at the top of the page, there are 17 forms. The images are static except for intake door image part which should change according to the selection made in the intake door type fields. We don't have the images there yet because they are not ready. This is work in progress. The features we need to be implemented are ; 1. "Save as PDF" button..so when someone has selected all the fields they want, they can save the full page as PDF (i.e. page with images and fields filled in) 2. "Print"...They can print the whole page after selections are made (images+field answers) 3. "Email" and the page (images+ answers filled in) is sent to the site admin We are on a tight deadline for this. Budget for creating all 17 forms is $125.

Hi, I would like to offer you something a bit different to what you described: How about a stand-alone PDF version of this form, including all the form fields and even the dynamic images? That way you could send out this file to people instead of having them go to your website in order to access it and they will be able to submit it back to you filled-in with all the required data by email, or just print it out and mail it... I specialize in creating custom-made, interactive PDF forms and could do this for you quickly and professionally within a couple of days. Let me know if you're interested in something like this and we could discuss it further (maybe under a new project, even). A little bit about me: I'm an Expert on both the Adobe and AcrobatAnswers forums and have a website dedicated to my custom-made tools for PDF files that you're welcome to check out (Google my handle-name to find it).
