I require a simple web form frontend that puts data into a mySQL database. Form's must be easily customisable through the source code, I don't mind editing the code in most languages. CSV upload into the tables of the mySQL must be supported for large batch entry. Front end must allow editing of a particular "entry" (row in the database) Export functionality to PDF using FPDF frame work or something similar so that PDF can be exported using a templated form or letter head. Export to CSV can be supported but not essential.

Dear sir, I am ready & confident to take this project. I have done similar tasks which include reading CSV file & also XL file and insert the data into SQL Server db.
