Build a Website

I need a very simple address book application which loads contacts from a database and displays the contact details. Related database will have only two tables, like addresses and cities. Design is simple; a listbox on the left and a textbox on the right. Listbox will display all contacts in the addresses table. Textbox will display the details of the selected contact from the listbox. Users will be able to select a contact from the listbox and click a remove button. This will remove the contact from the addresses database and refresh the listbox. There will be a dropdown list named as "filter by cities" which contains the city names from the cities table. Contacts with the selected city will be displayed when this filter is used. Empty value at the first index will display all contacts again. Finally, there will be an Add contact button which will be used to insert new contacts. This is for a simple exercise that will be used for education purposes. Good enough design and coding is what I am looking for. I will provide the guidelines as well.
