need a website redesign fast

[login to view URL] i want this site to be redesigned and a search form to b placed somewhere which will search doctors. "AMA doctor Search form" on home page like [login to view URL] but a modern looking form. i need to award the project today and to be delivered by 8 oct 2015. before awarding, i will demand a jpg design mockup based on my site. FYI: i dnthave any problem if you work with a premium theme from themeforest, but if you do, make sure you do not customise it, just use whateve is in theme. i dnt mind if u use demo link layout of those themes. Budget: not more than 200USD
