Customize existing Jplayer to play songs from a XML File with a feedback form

We have a html page with an audio player, Jplayer-ready. It's customized from a template so it's semi-functional. It's a single html page. We need it to load a playlist from a XML file, which we’ll export from iTunes, parse it, and use jplayer for basic audio functions (play, pause, skip, progress bar, volume, mute, playlist) The XML file has to be parsed and to be outputted as dynamic data inside the HTML. We also need a simple feedback form with a single field, using ajax, which will appear on the click of a button, and after the client clicks 'sent' it shows a success message. Again, all Ajax, no reloading please. Also, we just needed the functionality (form slide down on click, send message via php, form slide up, shows success message) - we'll style the form appearance later. Attached is a screenshot numbered with instructions, and marked where the dynamic data would go. And remember: please, not a single reload. We need it in a couple of days, tops. This shouldn't take an experienced PHP coder more than 5 hours, I believe.
