Create Invoice in PDF format using PHP LAMP

I need a PHP script to create an invoice in PDF format on Linux OS. Template invoice attached. I need exact same design to be created on-the-fly in PDF format. For development & testing purpose, the textual content should be taken from local variables. In real scenario, the data will come from mysql.
