Blackberry forms

I am interested in creating a Blackberry app that will allow the creation and entry of forms.? I have found a product called BerryVine Survey, but it seems to have been written quite some time ago and does not take advantage of new BES v5 technology.? I need the ability to use a template to create new forms, push them to blackberries using a BES v5 server, and after the forms are filled in by the users have the results emailed to multiple locations(one for storage, one to client).?
