Convert PSD file into HTML

I have a PSD file that I need turned into HTML and verified to work properly before the job is completed. The form and the buttons need to be created, however they do not need to be linked to anything. We will do that part. We just need the design and the functionality to work without anything being sent anywhere. This job will go to the applicant that can demonstrate the ability to complete this task cleanly and quickly, preferably having examples of past work to show. The job has no Javascript or anything fancy associated. There are about 65-70 elements in the PSD file.
