ID Card Generator in PHP CODEIGNITER

Need a PHP Codeigniter Code to design an ID card size of ATM card to be designed exactly as attached in the picture. Need dynamic insert that prints the card in *.jpg format in exactly the size of the ATM card [85.60 mm × 53.98 mm (3.370 in × 2.125 in) and rounded corners with a radius of 2.88–3.48 mm] Design needs to be exactly the same as mentioned in the attachment. Also all fields need to be dynamic. We just need a simple form with these fields when submited generates this card. Thanks
