Create Google Sheet form

We need to create a Google Sheet form that takes inputs from a user to produce a particular calculation. In summary: - There are 7 questions (5 dropdown, 2 multi choice) questions that cover three categories - Depending on the answers to each question, a numerical score is produced for each category - The numeral score for each category will fall within one of four pre-defined score ranges - A particular score range gets turned into a dollar amount - The final calculation is to aggregate the dollar amounts for the three categories to produce one dollar amount. You will: - design the Google Sheet as a Form - showing only the 7 questions and then results - (1) The final score range result for each category, (2) The Final dollar amount, (3) A High/medium/ Low result depending on the result of the Final Dollar Amount
