Meteor Framework: Create single form (tigger email & save to db)

Brief: 1. Create a online form in Meteor Framework (attached fields) - mobile friendly. 2. Save results to the database. 3. Send email with results via API (Sendgrid API). Specific fields: 'Event' field should be a searchable dropdown field linked to mySQL database. 'Your Signature' field should be an e-signature field.
