Need to Create a program to track exam candidates

I have attached a .pdf mock-up of what how I envision the site to work. I may have missed a few steps or there may be a feature that is needed after the script is tested. You can set up a test site on your server for me to test before releasing the files. You must be able to speak fluent English. You need to provide a video of the software working in order to have any money released. The money will not be released until you have shown software that is working. Do not ask for early release. This is not allowed by freelancer. Please ask as many questions as you can before bidding. You will need to release all files used to create the program. Coding must be commented out heavily so future adjustments will be easy. You must be willing to make adjustments during the development process to handle unforeseen issues.
