Simple Android Application for logging comments on received calls on server

Looking for a small android application that will 1. Automatically come up when a call arrives or goes out from the android phone. 2. It checks if the phone# is already in the server mysql database(on cloud) or on the backup locally(if internet not available). 3. If not present on server, it must add with a popup (screen mockup given). 4. It must ask and record 9 fields for new phone numbers: name(first and last), phone#1 with countrycode, phone#2 with countrycode, email id, Degree Preference (text field), A 256 char Comment field, Date/time and duration of call. 5. If there is no internet connectivity, it must save data locally and upload when internet access becomes available. 6. It must save even dropped and unanswered calls to the main server. 7. Server side database (mysql) is simple - has 2 tables, contact info table (name, country code and phone number, additional phone#1, 2, email id, Degree preference) and call log table(related to contact info table one to many, call start date and time, duration, status (normal completion or disconnect or did not pickup), A 256 char comment field. 8. When the call ends, it must find if the contact list on the phone has that number saved under a name and if not, it must ask for contact information to save to server. 9. All contacts saved on the phone and server through this application must have a prefix (2016) in it. 10. The prefix should be settable on the app (modifiable) 11. A photo must be settable on the top left corner and title must be settable 12. Need compilable working code at the end of the project. 13. Screen mockup given (can be made better) 14. Payment in phases.
