Small PHP Project (Very simple)

Good day, I require assistance with a educational site that im busy with. It is in the beginning stages so doesnt look like much, but require help with the last lesson. What I required: Use a PFBC class to build an online application to manage a TaskList application – see screen attached. The application must allow for the addition and deletion of items form a task list as well as to mark an item as complete. TaskList items are stored in a database. The application consists of these files:  [login to view URL]: adds an item to the list  [login to view URL]: creates the MySQL table for the application (This file will only be called once)  [login to view URL]: deletes an item  [login to view URL]: gets all items in the database  [login to view URL]: displays tasks and provides a form for adding an item to the list  [login to view URL]: changes the status of an item to complete  [login to view URL]: contains functions that add, get, update, and delete items from the database Lastly, name this [login to view URL] and upload it to the webhosting area that i will provide after project has been awarded.
