Build a Website

Need a simple small microsite that allows clients to log in, fill in forms and store entries in a database. The layout may need to be tweaked but currently designed already. Would like an easy way to organize the data, send in an email or pull results. Will possibly need to pass a variable to and from a video. Looking to develop a longterm relation with a new web professional. Prefer US based freelancers, or be sure you can speak and understand English very well otherwise. Thanks!
