Client Management solution for charitable organisation in Sydney

We are a youth charity in Sydney Australia. [login to view URL] We deal with young people aged 13 - 26 with Mental Health, Alcohol and Drug issues, housing applications and the like. We are looking to build a web-based application to better manage of Clients and their casework. I have built a database schema that I believe will support most of the functionality that we require.
