Client tracking web app

I need a PHP app that will allow users to enter tracking info for clients (name, phone, other details) and associate them with certain customizable categories of information. There must be user/group level security and ability to print reports of clients and whatever info is associated with them. The admin level users also must be able to insert new fields into the database (MySQL) which display on the client info page. See the attached jpg for details of the info to be stored. Users should be able to view the data historically, and should be able to report the data, selecting the type of data they want to see and a date range. Also, the users should be able to insert URL links into a list of documents that the client might want to have emailed to them. ## Deliverables Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. ## Deadline information User interface screens should be done by 1/3/02. Database should be ready by 1/30/02 and final program ready for pilot by 2/20/02
